What is it?

The purpose of this procedure is to choose a primary care doctor at the Benitatxell health centre. The form should only be used whenever you wish to change the primary care doctor at the health centre.

Who is it aimed at?

Any person who is insured or a beneficiary of the National Health System and is in possession of an individual health card issued by the regional ministry responsible for health matters.

The request for free choice of primary care doctor can be made by:

• The person who holds the right.
• The person authorised by the right holder. (download authorization form)
• The person who holds legal representation (in the case of minors who are not emancipated or persons who are legally incapacitated by court order to exercise their rights). In the case of minors under the guardianship of public administrations, the application may be made by persons who have been granted custody by virtue of a foster care or residential care order.

Requirements

Be insured or a beneficiary of the National Health System, in accordance with applicable regulations, and be in possession of the individual health card (SIP) issued by the regional ministry responsible for health.

Form of presentation

• Telematically
• In person: in the Benitatxell health centre. Also at any post office. In this case, it must be presented in an open envelope so that the stamp and date can be stamped on the application form and on the copy.

Resolution

The maximum period for resolving and notifying the decision shall be fifteen working days from the date on which the application is entered in the register of the competent body for processing. If no express decision has been made within this period, the application shall be deemed to have been approved.
